The name on the front will be "ChiroCongress" - the player name on the back will be your state (i.e. "Michigan) - The number on both sides will be "02".
The size guide is only accounting for the front side measurement as shown the diagram. Example: armpit to armpit. For the full circumference double up the measurement.
Please note, these measurements are ONLY one dimensional (i.e. measure from one side of chest to the other and not circumference).
