With that understanding, you voluntarily assume all risk that you may be exposed to or infected by COVID-19, which could result in injury, illness, disability, or death to you or those with whom you come into close contact.
You further acknowledge and agree to accept sole responsibility for any claim, including, but not limited to, illness, injury, disability, or death of you or your immediate contacts (“Claims”). You release, discharge, covenant not to sue, and hold harmless the Illinois Chiropractic Society, and any individual or entity acting on its behalf, from liability for any Claims arising out of or relating to COVID-19, whether a COVID-19 infection occurs before, during, or after participation in any Illinois Chiropractic Society event.
You agree to abide by rules by or instructions of the Illinois Chiropractic Society, the venue where the event is held, or any government authority, with respect to any required safety mitigations and sanitation guidelines.
You further agree not to attend the event if you are diagnosed with or suspect that you may have been infected by COVID-19 or if you exhibit any symptoms of COVID-19. In the event you become ill or suspect you have been infected with COVID-19, you agree to immediately discontinue your participation in the event and alert the Illinois Chiropractic Society via e-mail or telephone.
If you do not agree to or accept the foregoing, do not register for the Illinois Chiropractic Society’s live event, instead, please register for the Virtual Simulcast event. Failure to agree to or abide by any of the foregoing may result in denial of entry or removal.
